How does an anthology get started?

How does something like Flight get off the ground (PUN!)? Do you approach the authors first, get the whole thing together, and take it to a publisher? Or do you get the concept first, go to the publisher, and then approach the authors with the publisher's green light? Do you pay the first volume's a...
